1. A holder for supporting a rod to be embedded in concrete, comprising:
a) a unitary body including a first opening configured to receive an end portion of the rod, the body including an underside configured to be positioned on a form board; and
b) a circumferential flange disposed at a bottom portion of the first opening, the circumferential flange including a second opening located below the first opening, the second opening configured to receive a screw or nail attaching the body to the form board, the screw or nail including a head portion supported on the circumferential flange.
