1. A polymer-coated substrate, comprising:
a substrate;
a silane compound layer formed on a surface of the substrate; and
a polymer layer formed on the silane compound layer,
wherein the polymer layer is formed of:
(i) at least one selected from the group consisting of polymers represented by the following formula (I):

where R51 represents a hydrogen atom or a methyl group; R52 represents an alkyl group; p represents 1 to 8; m represents 1 to 5; and n represents the number of repetitions and n is 15 to 1500, or
(ii) a copolymer of at least one hydrophilic monomer selected from the group consisting of compounds represented by the following formula (II) with an additional monomer:
