1. An eye health supplement dosage form, comprising:
1 part by weight zeaxanthin;
5 parts by weight lutein;
16.75 parts by weight vitamin E;
3.75 parts by weight zinc;
25 parts by weight curcumin; and
30 parts by weight omega-3 oils including docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA).
