1. A surgical instrument comprising:
an outer sleeve;
spaced apart tabs extending from a distal end of the outer sleeve such that the tabs are flexible relative to the outer sleeve;
an intermediate sleeve positioned within the outer sleeve;
a shaft coupled to the intermediate sleeve, the intermediate sleeve comprising a threaded portion disposed in a threaded recess of the shaft; and
an actuator comprising opposite lever handles each having a first end that is coupled to the shaft and a second end that is coupled to the outer sleeve via a linkage such that simultaneous rotation of the lever handles relative to the shaft and the sleeves translates the intermediate sleeve relative to the outer sleeve.
