1. A method comprising:
receiving a scan of the patient's dentition and data indicating a sleep apnea diagnosis;
generating a treatment plan to move the patient's teeth in a series of stages from a first arrangement towards a second arrangement and to advance a lower jaw of the patient anteriorly during treatment; and
determining, with aid of one or more processors, for at least a first stage of a treatment plan, a location of a first advancement structure arranged to interact with an opposing jaw of the patient so as to provide a force to displace the lower jaw anteriorly relative to the upper jaw, an arrangement of a first plurality of cavities to distribute the force to a first plurality of teeth of the patient's teeth, and a location of a coupling of the first advancement structure to mate with a coupling of a second appliance shell.
