1. A system, comprising:
an implant having a unitary body, the implant including:
an intramedullary portion sized and configured to be received within a first bone portion, the intramedullary portion defining at least one first aperture having an aperture axis that is disposed at an oblique angle with respect to a longitudinal axis defined by the intramedullary portion, the at least one first aperture sized and configured to receive a first fastener for securing the intramedullary portion to the first bone portion; and
an extramedullary portion configured to abut a surface of a second bone portion, the extramedullary portion defining a plurality of second apertures, at least one second aperture of the plurality of second apertures sized and configured to receive a second fastener for securing the extramedullary portion to the second bone portion, and
a targeting guide having a body configured to engage the implant, the body defining a third aperture and a fourth aperture, the third aperture defining an axis that is disposed at an angle with respect to an axis defined by the fourth aperture,
wherein the targeting guide is configured to engage a wire for rotating the second bone portion about a longitudinal axis of the second bone portion when the intramedullary portion of the implant is disposed within the first bone portion.
