1. A system for managing an individualized cardiac rehabilitation plan for an ambulatory cardiac rehabilitation patient, comprising:
an externally worn device configured to monitor cardiac activity of the patient, and
a server in communication with the externally worn device, comprising
a non-transitory computer-readable storage medium, and
at least one server processor operably coupled to the computer-readable medium, wherein the at least one server processor is configured to
receive one or more inputs from a healthcare provider related to the patient,
develop a patient-specific cardiac rehabilitation plan based on the one or more inputs from the healthcare provider, the patient-specific cardiac rehabilitation plan comprising a plurality of exercise sessions, wherein each given exercise session comprises one or more physical activities for the patient to perform during the given exercise session,
transmit the patient-specific cardiac rehabilitation plan to the externally worn device,
receive patient data from the externally worn device after the externally worn device has started administering the patient-specific cardiac rehabilitation plan to the patient,
record one or more status identifiers for each past exercise session based on the received patient data,
wherein recording the one or more status identifiers for each past exercise session comprises recording a completed status identifier in response to the patient completing the given past exercise session or a non-completed status identifier in response to the patient not completing the given past exercise session,
wherein the one or more status identifiers further comprise an over-performance status identifier recorded in response to the patient exceeding at least one requirement of the given past exercise session by at least a predetermined over-performance amount or an under-performance status identifier recorded in response to the patient satisfying at least one requirement of the given past exercise session and not meeting at least one additional requirement of the given past exercise session by at least a predetermined under-performance amount,
adjust the patient-specific cardiac rehabilitation plan based on the one or more recorded status identifiers,
wherein adjusting the patient-specific cardiac rehabilitation plan based on the one or more recorded status identifiers comprises performing at least a first adjustment based on an over-performance status identifier recorded for a past exercise session or performing at least a second adjustment based on an under-performance status identifier recorded for a past exercise session, and
transmit the adjusted patient-specific cardiac rehabilitation plan to the externally worn device;
wherein the externally worn device is further configured to
administer the patient-specific cardiac rehabilitation plan and the adjusted patient-specific cardiac rehabilitation plan;
monitor at least the patient's heart rate during a given exercise session;
determine whether the patient's monitored heart rate is below a minimum heart rate, above a maximum heart rate, or between the minimum heart rate and the maximum heart rate; and
provide an alert based on the determination of whether the patient's heart rate is below the minimum heart rate, above the maximum heart rate, or between the minimum heart rate and the maximum heart rate.
