1. An intervertebral cage configured to be inserted into an intervertebral disc space defined between first and second vertebral bodies, the intervertebral cage comprising:
a cage body including an upper plate configured for placement against an endplate of the first vertebral body, and a lower plate configured for placement against an endplate of the second vertebral body;
an upper cantilever arm having a proximal end and a distal end, and an upper body that is supported at the distal end of the upper cantilever arm, wherein the upper body defines a sloped upper bearing surface;
an actuator disposed in the cage body, the actuator having an upper ramped surface, wherein the actuator is configured to be driven in an actuation direction from an initial position to an actuated position, which causes the upper bearing surface to ride along the upper ramped surface, thereby driving the upper body away from the lower plate;
wherein the actuator is captured by the cage body without any external fixation screws when the actuator is in the initial position and as the actuator moves from the initial position to the actuated position, and
wherein the actuator is configured to receive a drive force directly from a driver instrument that is separate from the cage body, and the drive force causes the actuator to be driven in the actuation direction from the initial position to the actuated position.
