1. An implantable port device comprising:
a housing made of a biocompatible material;
a fluid reservoir within the housing;
a self-sealing cap covering the fluid reservoir;
a catheter connector in fluid communication with the fluid reservoir;
a battery disposed within the housing;
a disk-shaped circuit board arranged to circumferentially surround the fluid reservoir;
a plurality of sensors configured to detect physiological data while the device is implanted within a body of a patient, comprising a heart-rhythm sensor and an electromagnetic sensor configured to detect data characteristic of impedance, wherein at least one of the plurality of sensors are arranged directly onto the disk-shaped circuit board;
an LED device arranged on the disk-shaped circuit board;
a data storage component disposed within the housing, the data storage component configured to store, while the device is implanted within the patient, physiological data detected via at least one of the plurality of sensors; and
a wireless communication component configured to transmit, while the device is implanted within the patient, physiological data to one or more external computing devices.
