1. A device implantable into the eye, comprising:
an elongate duct portion having a longitudinal axis, the duct portion defining two lumens that extend parallel to one another from a distal end to a neck spaced longitudinally from the distal end; and
a second portion including tubular segments that extend or flare outwardly from the neck opposite one another relative to the longitudinal axis and form a tubular loop that surrounds an interior area, wherein the tubular segments of the second portion that form the tubular loop define an enclosed lumen with a plurality of separate and distinct outlets or openings spaced apart from one another that are in fluid communication with the two lumens of the duct portion to permit fluid to flow through the device;
wherein the elongate duct portion and the tubular loop are formed together as a unitary structure that forms a closed loop prior to implantation of the device.
