1. A surgical instrument comprising:
a first member including a sleeve defining an axis and a drive coupled to the sleeve, the drive being engageable with a first mating surface of a head of a bone fastener and coupled to the sleeve such that rotation of the sleeve in a first direction about the axis rotates the drive in the first direction about the axis and rotation of the sleeve in an opposite second direction about the axis rotates the drive in the second direction about the axis;
a second member rotatably disposed in the sleeve and including a screw having a threaded surface connectable with a second mating surface of interior walls of a receiver of the bone fastener, and
a thumbwheel coupled to the second member and configured to rotate the second member relative to the first member about the longitudinal axis,
the sleeve comprising a first end with bifurcated arms and an opposite second end, the drive being coupled to the second end, the bifurcated arms defining a cavity therebetween, and
the thumbwheel being positioned in the cavity defined by the bifurcated arms.
