1. An insertion instrument configured to insert an implant into an interspinous process space or intervertebral space, the insertion instrument comprising:
an elongated main body having a proximal handle portion defining a central passage and a distal locking portion that selectively couples to the implant;
a plunger slidably engaged in the central passage to fix the implant to the elongated main body;
a proximal drive shaft operatively engaged by the proximal handle portion;
a distal spike cap drive concentrically located partially around the plunger and the proximal drive shaft, the distal spike cap drive having a socket end configured to operatively engage the implant; and
one or more complementary slots comprising a radial portion disposed on the proximal drive shaft, the one or more complementary slots configured to receive pins disposed on a proximal portion of the distal spike cap drive.
