1. An implant assembly comprising:
a proximal head portion;
a substantially-spherical cradle having a central bore configured to receive a spinal fixation element and opposed flat portions on an external portion thereof, the cradle seated within the proximal head portion so that the opposed flat portions bind the cradle against the proximal head portion;
a locking mechanism receivable in the proximal head portion; and
an anchor portion extending distally from the proximal head portion, the anchor portion configured to engage with bone;
wherein actuation of the locking mechanism is effective to fix an orientation of the cradle relative to the proximal head portion.
