1. An apparatus comprising:
a sheath including a flexible elongated body extending from a proximal end to a distal end and configured to deliver an elongated medical instrument to a target location within a patient;
an insertion device configured to position a distal end of the sheath adjacent the target location, the insertion device including a lumen terminating at a head, the lumen configured to deliver the sheath to the target location along an extension axis, wherein the head includes an ultrasound transducer; and
a closed sheath tip disposed on a distal end of the sheath, the closed sheath tip having a solid end surface, wherein the solid end surface is configured to be pierceable by the elongated medical instrument having at least one of a pointed end and a sharpened edge, and wherein the sheath defines therein a second lumen configured to convey the elongated medical instrument therethrough such that, when the closed sheath tip is presented at a tissue surface, the solid end surface of the closed sheath tip is configured to distribute force of impact with the tissue surface across the solid end surface and to permit the at least one of the pointed end and the sharpened edge the medical instrument to pierce the solid end surface and extend therethrough toward the tissue surface.
