1. An implant for placement between spinous processes of a spine comprising:
a spacer comprising a first end, a second end with a second end terminal edge, and a sidewall therebetween with an elongated slot, wherein the elongated slot passes through the sidewall into an interior cavity defined within the sidewall of the spacer,
a first extension having a superior lobe, an inferior lobe, a main body, and an aperture in the main body in cooperation with the interior cavity of the spacer, the first extension projecting outwardly from the first end of the spacer, wherein the spacer is fixed to a medial surface of the first extension at the first end of the spacer; and
a second extension having a superior lobe, an inferior lobe, and a span connecting the superior and inferior lobes of the second extension, the second extension having a post extending into the elongated slot of the spacer to form a first mechanical lock to connect the second extension to the spacer, wherein the second extension is able to pivot about the spacer relative to a longitudinal axis extending through the spacer prior to the second extension being locked to the spacer, and wherein a medial surface of the second extension is slidable onto the second end with the second end terminal edge of the spacer,
wherein the first extension is fixed to the spacer by means of a second mechanical lock, and wherein the spacer includes at least one protrusion extending radially outward from the spacer to cooperatively engage a lock groove portion of the second mechanical lock.
