1. An insulin monitoring and delivery system, the system comprising:
one or more processors, one or more non-transitory computer-readable storage devices, and program instructions stored on at least one of the one or more storage devices for execution by at least one of the one or more processors, the program instructions, when executed by the one or more processors, causes the one or more processors to:
detect presence of a first condition based on comparing feed forward insulin action with a filtered insulin action involving a Kalman filtering technique,
detect presence of a second condition based on an insulin need, wherein the insulin need is based on comparing insulin that should be on board (ISOB) with insulin on board (IOB), wherein the IOB is based on an amount of insulin active and/or an amount of insulin to be active, and wherein the ISOB is based on an estimation or a prediction of a glucose level;
upon detecting presence of the first condition or the second condition, classify the detected first condition or second condition into a fault level indicative of a glycemic state; and
generate a command signal for use by an insulin delivery device to adjust an insulin infusion rate and/or insulin infusion amount based on the fault level.
