a first portion of the tubular implant configured to be attached proximate a sacrum;
a transition region extending from the first portion;
a second portion of the tubular implant extending from the transition region monolithically and including a first section and a second section and two slits provided laterally in the second portion separating the first section from the second section at a proximal end, the two slits extending from the transition region to the proximal end of the second portion, the first section having a first edge that extends linearly and parallel to a first edge of the second section, the first section having a second edge that extends linearly and parallel to a second edge of the second section, the first section being configured to be placed in a stretched configuration and retain such configuration upon the application of heat to the first section; and
a lumen extending through the first portion, the transition region, and the second portion,
wherein the first section is configured to be attached to an anterior vaginal wall, and the second section is configured to be attached to a posterior vaginal wall,
