1. A coupler device configured for use with a forward viewing endoscope having a working channel with a distal end, a light and a camera lens, comprising:
a main body comprising a distal end and a proximal end configured for removable attachment to a distal end portion of the endoscope by a sealed connection;
a flexible working channel extension within the main body, the working channel extension having an open distal end and a proximal end that is configured for attachment to the distal end of the working channel of the endoscope;
a sheath extending through the main body of the coupler device, wherein the sheath is coupled to the flexible working channel extension and configured to receive a cable therethrough for moving the working channel extension from a first position, wherein the proximal end of the working channel extension extends from the distal end of the working channel of the endoscope substantially parallel to a longitudinal axis of the endoscope, to a second position, wherein the proximal end of the working channel extension extends from the distal end of the working channel of the endoscope at an angle to the longitudinal axis of the endoscope; and
wherein the coupler device is configured to allow a view of tissue located distal to a distal end of the endoscope.
