1. One or more non-transitory media storing instructions executable by a processor to initiate operations comprising:
generating, while displaying a primary medical record for a patient, a graphical user interface that presents a template usable for creation of:
(a) a subjective assessment, of the patient, by a clinician; and
(b) one or more objective medical factor indicators, corresponding to one or more objective medical factors in the primary medical record, associated with the subjective assessment and considered by the clinician;
receiving first input for the creation of the one or more objective medical factor indicators, and second input for the creation of the subjective assessment, via the graphical user interface;
embedding, in a user-interactive medical consultation report corresponding to the template, one or more links from the one or more objective medical factor indicators to one or more corresponding objective medical factors in the primary medical record; and
storing an integrated medical consultation report, with the subjective assessment and the embedded one or more links, in the primary medical record as a unique document.
