1. A method, comprising:
receiving, in real-time at a healthcare command system, data from a plurality of sources through one or more networks and related to a developing health condition within a community, wherein at least one of the plurality of sources comprises community surveillance data that covers a community and at least one community sensor that receives data covering a public space within the community;
identifying a geographic region of the community based upon a geographic location corresponding to the data;
identifying the developing health condition for the community by correlating the data for the geographic region containing the community to potential health effects, wherein the identifying comprises identifying, from the data, one or more symptoms of the developing health condition and correlating the one or more health symptoms to at least one potential diagnosis of the developing health condition; and
performing at least one action to respond to the developing health condition, wherein the at least one action comprises alerting at-risk patients based upon the potential health effect and connecting the at-risk patients with a health professional to address the developing health condition for the at-risk patients, wherein the connecting comprises identifying a communication medium for a communication between each of the at-risk patients and the health professional based upon a determined treatment plan for a given of the at-risk patients and initiating the communication for the communication medium identified.
