1. A computer-implemented method, comprising:
retrieving, from an electronic health record, an information for a subject, the information comprising a condition of the subject and a geographic location of the subject;
identifying a patient cohort for the subject, based on the information for the subject;
retrieving, from the electronic health record, a list of chronic conditions for the patient cohort sorted by an incidence level in the geographic location;
evaluating a determinant factor in the condition of the subject, based on the list of the chronic conditions;
identifying one or more resources for resolving the determinant factor available in the geographic location;
identifying a healthcare success factor based on an availability of the one or more resources for the subject and an inverse correlation between a number of patients in the patient cohort and a number of co-morbidities affecting the number of patients;
identifying an expected condition for the subject based on the healthcare success factor and a historic performance with the patient cohort associated with the condition of the subject;
identifying a plurality of actions to be performed by the subject to improve the expected condition for the subject;
receiving, via a user interface of a client device associated with a provider, a selection of a first action of the plurality of actions; and
directing the provider, via an electronic communication, to request an access to a resource of the one or more resources, the resource including a facility supporting the first action.
