1. A system for creating an on-the-fly index, the system comprising:
a viewer process, for a health information exchange (HIE), that requests clinical data from an interoperability network of electronic health records (EHRs), utilizing contextual information and activity indicators for clinicians and patients, wherein the requesting triggers queries at an index engine;
the index engine that maintains an index for a subset of one or more patients and satisfies the queries for the subset of one or more patients; and
an in-memory document cache that, in memory using a stateless infrastructure and in response to the queries, stores patient-specific document sets, allows retrieval of individual documents or patient-specific document sets for a desired patient, or retains documents.
