1. A device for forming a void in concrete, comprising:
a single base having a square shape with opposing flat and planar surfaces and a plurality of holes through the opposing flat and planar surfaces, each of the holes having a size and a shape to receive a fastener for coupling the base to a piece of formwork, the base having a width; and
a single elongate member extending perpendicular from the base and centered with respect to the base as a single, unitary, integral component with the base, the elongate member having a shape and a size to create the void when the concrete cures around the elongate member and the elongate member is removed from the cured concrete, the void having a size and a shape to receive a reinforcing bar, the elongate member having a length greater than the width of the base and wherein the elongate member is a solid body having a constant circular cross section along an entire length of the elongate member.
