1. A method for preparing a modified conjugated diene-based polymer, comprising:
polymerizing a conjugated diene-based monomer in the presence of a rare earth metal catalyst composition to prepare an active polymer;
adding sulfur chloride and an additive comprising a compound represented by Formula 1 to the active polymer; and
modifying the active polymer in the presence of the sulfur chloride and the additive:

Rx is an aliphatic hydrocarbon group of 1 to 30 carbon atoms, an alkylaryl group of 7 to 30 carbon atoms, or an arylalkyl group of 7 to 30 carbon atoms,
Ry is a hydroxyl group (—OH), an aliphatic hydrocarbon group of 1 to 30 carbon atoms, an alkylaryl group of 7 to 30 carbon atoms, or an arylalkyl group of 7 to 30 carbon atoms, and
n is an integer of 1 to 20.
