1. A method for preventing transmission of an incorrect copy of a record to a distributed ledger system, the method comprising:
receiving, by a processor, a first file, the first file comprising:
recorded audio and/or video;
a description of a subject matter of the record; and
an authorization to cause the record to be transmitted to the distributed ledger system;
sending, by the processor, at least the description of the subject matter of the record to a device associated with a first entity;
receiving, by the processor, a confirmation that the description of the subject matter is correct;
receiving, by the processor, a confirmation that a second entity that controlled production of at least a portion of the first file has permission to authorize causing the record to be transmitted to the distributed ledger system;
responsive to receiving the confirmation that the second entity has permission to authorize causing the record to be transmitted to the distributed ledger system, causing the record to be transmitted to the distributed ledger system; and
responsive to a failure to receive the confirmation that the second entity has permission to authorize causing the record to be transmitted to the distributed ledger system, preventing transmission of the record to the distributed ledger system.
